Personal Care
Unforeseen circumstances can make the simplest activities of daily living (ADL) challenging. Safe and Prudent provides personal care services allowing our clients to maintain their lifestyle in the comfort and familiarity of their homes.
Below are examples of personal care support services we offer, which can be customized to meet your needs:

Skilled Nursing Care
Safe and Prudent provides skilled nursing services to our clients by professional nurses on a full, part time or per visit basis under the care and direction of your physician. We monitor the delivery of these services and use evidenced based care to improve the outcome with specific quality of life or health status improvement goals in mind.
Below are examples of clinical services our staff of professional nurses can provide for our clients:
Overall health monitoring, Administering medications, Monitoring vital signs, Caring for wounds, Blood or other lab work, Diabetes care, Catheter care, Ostomy care, Colostomy care, Tube feeding, Ventilator maintenance and care, Technology dependent care,and Catastrophic care.
Long-Term Chronic Care
The goal of our chronic care programs is to prevent acute exacerbations leading to hospitalization. We focus on keeping the patient well and medically stable. Safe and Prudent offers chronic disease specific care management programs for our clients with a chronic condition such as diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and many other debilitating conditions. We also provide in home services for individuals in need of care 24 hours a day, 7 days a week.
Therapy
Safe and Prudent provides therapeutic services to clients under the care and direction of their physician. Safe and Prudent makes it possible to receive therapy services in the comfort of our clients’ homes in accordance with their physician’s protocol.

Companion & Socialization Services
These services in particular are based on the creation of a solid client/companion relationship, which is why all of the Safe and Prudent companions must exemplify integrity, loyalty and empathy. Our companion and socialization services focus on helping people to re-engage in life. We select companions based on their qualifications, interests and unique characteristics in an effort to provide our clients with an ideal and trusted companion.
Our companions participate in a number of activities based on the needs and interests of our clients, including:
Meal planning and preparation, walking and exercise, transportation for errands, events and appointments light housekeeping and organization, pet and plant care, reminders of medication, events, birthdays, respite care, and many more personalized services.
The main objective of our companion and socialization services is to establish a strong personalized relationship with our home care clients in order to enhance their quality of life and eliminate isolation.
Short-Term Assistance
Safe and Prudent offers short term assistance services for clients in need of home care for a specific duration of time. Whether you need 24 hour a day, seven days a week care for a few months or you need a caregiver just a few hours a week, Safe and Prudent can help.
Below are common instances when short term assistance services can benefit our clients:

A client’s overall well-being and elevating their quality of life is our top priority. We listen, attune and connect in everything we do. We begin our home care process with a Registered Nurses’ comprehensive assessment of a client’s physical, social, mental and safety needs. We further evaluate the potential environmental hazards and other risks in a client’s home, which may impact the outcome of care. All risks are mitigated to the extent possible through a customized service plan.
The initial assessment of our clients serves as the basis for the creation of a home care service plan, which is designed and supervised by a registered nurse. Every service plan is discussed with family members and medical personnel, with a main objective of improving the quality of life for our client. All of our standards of performance are designed to provide the highest quality home care services possible.
